Homecare Package Manager Makes a referral to us.

When the referral is received we try to make contact in the following hours to schedule a call with our new patient or their HCP manager

Welcome Call

We will call the client to welcome them and discuss the referral. This can be completed by their HCP manager also.

Initial Consultation

One of our community Physiotherapists will visit you and listen to and discuss your conditions, issues and limitations.

Assessment

During your initial and follow up consultation your Physio will complete a thorough assessment of your health, fitness and / or injury.

We are then able to create a brief one page plan of the health goals we agree are most important to work on together in your sessions

Your One Page Health & Goal Plan

We share the agreed goal plan with our client and their Home Care Package manager & the family if they choose.

The idea is to create a network that is positive and aligned fostering a team environment to establish what we are working on, how long it may take and the costs involved.

Individualised Physiotherapy, Exercise and goal achievement

Once we have agreed on the plan, your Physiotherapist will guide your treatment and sessions working with you towards your goals!
COVID-19 Update - We Are Open

Our Clinic has been classified as an essential service to the community. We assist by trying to keep people healthy as possible and out of surgeries and decrease hospital admissions. At Inertia Health Group we are working hard to keep our patients and team members as safe as possible.

Due to our strict infection control procedures we have been deemed a low risk clinic- we have all completed the government COVID -19 Infection Control Risk Course and have implemented new waiting room procedures as well as intensified our cleaning procedures completing these regularly through the day. We have also implemented a virtual health service for those who require advice or assistance and cannot attend the clinic.
