What is a Home Care Package?
Federally funded packages to help seniors access in-home care services
Funding amounts are varied depending on the level of care required from the independent needs assessments.
Funds can be used for a variety of services such as Exercise Physiology.

One of our community Exercise Physiologists will visit you and listen to and discuss your conditions, issues and limitations.
How Do Exercise Physiology Homecare Packages Work?
Seniors who are eligible for a Exercise Physiology homecare package sign up with a homecare package provider. The homecare package provider then assess the clients needs and requirements and then arranges the appropriate referrals to service providers.
Home care package managers and/ or the clients can coordinate the Exercise Physiology assessments, goals and services with the Inertia Health Group.
Inertia Health Group Exercise Physiologists work together with our patients and their families to ensure a good connection and we work towards set goals together.

When the referral is received we try to make contact in the following hours to schedule a call with our new patient or their HCP manager
Welcome Call
Initial Consultation
One of our community Exercise Physiologists will visit you and listen to and discuss your conditions, issues and limitations.
Assessment
During your initial and follow up consultation your Physio will complete a thorough assessment of your health, fitness and / or injury.
We are then able to create a brief one page plan of the health goals we agree are most important to work on together in your sessions
Your One Page Health & Goal Plan
We share the agreed goal plan with our client and their Home Care Package manager & the family if they choose.
The idea is to create a network that is positive and aligned fostering a team environment to establish what we are working on, how long it may take and the costs involved.
